Care co-ordinators help to co-ordinate and navigate care across the health and care system, helping people make the right connections, with the right teams at the right time. They can support people to become more active in their own health and care and are skilled in assessing people’s changing needs. Care co-ordinators are effective in bringing together multidisciplinary teams to support people’s complex health and care needs.

They can be an effective intervention in supporting people to stay well particularly those with long term conditions, multiple long-term conditions, and people living with or at risk of frailty.

4YP

Montpelier Health Centre is a 4YP Doctor’s Surgery. Many local doctors’ surgeries take part in the 4YP scheme. This means that young people can come to us for any concerns about sex and contraception, and they can be sure they’ll be treated with respect.  Also, the service is confidential, friendly and free and the young people don’t even need to be registered at the surgery.

We offer:

• Chlamydia self testing kits
• Contraception
• Emergency Contraception
• Free condoms & c-card scheme
• STI (Sexually Transmitted Infection) testing & treatment
• Pregnancy testing

…as well as other health services.

We also want to encourage our registered patients to use our service more often especially in relation to sexual health and Chlamydia self testing.

• Chlamydia is the most commonly diagnosed sexually transmitted infection (STI) in the UK; affecting both men and women. Most people who have it will have no symptoms, without a test you will probably not know anything is wrong.
• If you or your sexual partner are left untreated it can cause infertility (not being able to have children) and long term pelvic pain.
• It can be easily treated with antibiotics.

Young people can make an appointment, drop in for a 4YP appointment/Teen health check or just pick up a chlamydia self testing kit.

We offer a travel consultation and will only administer the free NHS travel vaccines:

• DTP (diptheria/tetanus/polio)
• Hepatitis A
• Typhoid
• Cholera

Many people will be travelling to countries where they will also require vaccinations that are not available on the NHS and a private travel clinic appointment will be required:

• Hep B
• Japanese encephalitis
• Meningitis
• Rabies
• Yellow Fever

Contact the surgery as early as possible and ask for a ‘Travel Appointment’ You will then be sent a  ‘Pre-travel questionnaire’ text OR email depending on your preference. If you are unable to complete the questionnaire electronically, please collect a paper travel form from reception. This must be completed and returned to the surgery prior to attending for the appointment so that vaccination requirements can be planned. One questionnaire needs to be completed per family member as it forms documentation in individual clinical notes.

Social Prescribing referrals have ceased currently, however existing referrals are being worked through. Should you have any queries regarding social events and community based projects, please contact our care coordinators.

Physio Direct provides a physiotherapy consultation and advice service over the telephone with a senior chartered physiotherapist. 

Paramedic Practitioners (PPs) are Paramedics with additional education and training, equipped for greater patient assessment and management skills. They can diagnose a wide range of conditions and treat many minor injuries and illnesses. At MHC they support the Duty Dr in triaging patients who submit online requests and those who need same day attention. They may also see patients for more routine matters that match their skillset. Our PPs all receive regular supervisions with an experienced GP Partner.

Off the Record Bristol (OTR) is a mental health social movement by and for young people aged 11-25 across Bristol and South Gloucestershire.

OTRs provides mental health support and info to young people, and all services are free, confidential and self-referral, helping to combat mental and emotional health needs such as depression, anxiety, self-harm, OCD and body image.

OTR works in schools, colleges, universities and youth clubs to promote good mental health and provide tips and tricks to nurture positive wellbeing.

One of OTR’s projects is The Resilience Lab, a stress-busting group workshop which shares guidance on how to deal with life’s ups and downs, and this sits alongside other offers including Nature Works (horticulture-based drop-in groups in an allotment space), Corner Man (a collaboration with boxing gym Empire), and informal groups such as Yoga and Book Club.

OTR also provides specialist support for anyone identifying as LGBTQ+ (Freedom), experiencing self-harm (hARMED), or issues around body image (Shameless).

OTR sees mental health much like physical health – it needs to be looked after and maintained. Young people can sign up for OTR’s various projects online at otrbristol.org.uk, or drop-in to one of OTR’s Hubs – an informal space to learn more about what OTR does and how to look after mental health.

Minor surgery is appropriate for simple lesions such as skin tags, skin cysts, lipomas and moles where they are causing discomfort due to the location. Please see a GP to discuss the appropriateness of minor surgery and if it is available at MHC currently.

Blood Tests

If you have been asked by one of our clinical team to have a blood test you should speak to a member of our Health Navigation Team to arrange this.

The Health Navigator will be able to advise you of the next available appointment. Please be aware that most blood tests need to be carried out in the morning due to courier collection times.

Test Results

If you have been asked to ring the practice to find the results of your investigatory tests please be aware that these are only available after 2pm, at least five working days after the test was carried out.

Repeat Tests

There may be occasions when you are asked to provide a repeat sample for investigatory tests, this does not mean there is cause for concern. There are various reasons why repeat samples may be required including:

• Original results inconclusive
• Original sample too small
• Original sample labelled incorrectly

A member of our team will be able to advise you of the reason why a repeat has been requested.

The community based Urology service, provided by GP Care, aims to keep your clinical consultation and diagnostic testing within one visit.

The doctor will refer you to the Urology service if they think it would be beneficial for you to receive further investigations and a specialist opinion.
