The community nursing team is provided by Sirona Care and Health.
The service is available to housebound individuals and temporarily housebound aged 18 and over who are registered with a GP surgery in Bristol. This includes people who live at home, in supported housing or in a residential home. Individuals who live in nursing homes are not eligible because their nursing care is already provided.
Community nursing teams provide a wide range of care and support to housebound and temporarily housebound individuals who are over 18. This includes:
- Coordinating care for individuals with long-term conditions or complex health needs
- Administering medication and supporting self-administration
- Dressing and managing wounds
- Providing Catheter Care in Clinics and within the individuals home
- Providing Wound Care for housebound individuals after surgery
- Supporting issues around continence
- Caring at end of life
The team consists of healthcare professionals, including district nurses, community nurses, healthcare assistants, assistant practitioners, administrators and advanced clinical practitioners.
To access this service you can be referred by your GP or another healthcare professional. For people who have been discharged from the service, they can self-re-refer or call for clinical advice by calling the Single Point of Access Team (SPA).

Full range of contraception options, menopause and hormone replacement therapy, and general gynaecological advice.
We have a highly skilled team of nurses, who are fully trained in procedures such as cervical cytology, ear syringing, immunisations and venepuncture.
They also run chronic disease management clinics for patients with Asthma, Diabetes and Coronary Heart Disease, offer advice and immunisation to patients travelling abroad, give support to patients wishing to stop smoking and deliver a comprehensive wound management programme.
The Treatment Room is open every weekday and you can make an appointment by phoning or calling in at Reception.

- NHS Health Checks for all patients over 45 years of age
- Blood sampling, specimen collection (e.g. a variety of swabs, urinalysis, pregnancy testing, sputum and stool sampling).
- Wound management (including wound dressing, suture removal, management of leg ulcers and care of traumatic wounds).
- Ear syringing
- Administration of Depot injections
- Blood pressure monitoring and 24 hours ambulatory BP monitoring
- Baby Clinic (covering childhood immunisations)
- Foreign Travel Service
- Smear Clinic
- Chronic disease management of Asthma, Diabetes, Heart Disease and COPD

There is a Podiatry service available through the PCN. The doctor will refer you if they think that this service will be beneficial for you.
Foot assessments and ongoing care for patients with foot problems who have an at risk medical status, i.e. people with diabetes, rheumatoid arthritis, peripheral arterial disease, people who are taking Warfarin and steroids, people who are immunocompromised, e.g. receiving chemotherapy, HIV and people who have neurological deficit to their lower limbs.
This service also offers assessments and ongoing care for people who have no medical risk and these patients are placed onto curative/short term treatment plans.
